- Priority Scheduling - Guarantee turnaround times or rush dates by
contacting us in advance of case arrival. Schedule your cases by Fax (617-924-8163), E-mail
annm@smilevision.net or leave a
phone message 800-634-3480 Ext. 14. We will phone you promptly if we cannot fulfill your request. Be
sure to include arrival and due dates, patient name, number of units and type of restorations.
- Cases not scheduled in advance - Turnaround times may vary depending upon our
workload. Include your desired due date on your RX and we will make every effort to meet it. You will be
contacted well in advance if we cannot meet the requested date.
- Rush cases - There is a 10% additional fee for each workday less than the
normal turnaround time. Contact us in advance to schedule rush services.
- Imaging rebate - We credit 50% of the imaging fee with submission of six or
more units for 3-D replica and templates. We also offer an additional 50% imaging credit for cases of
six or more restorative units when they are received in our laboratory.
- Payment terms - Credit card information is kept on file. It is charged as
cases are shipped.
- Shipping - Call us at 800-634-3480 X 10 for a UPS pickup. We supply boxes
and offer complementary shipping to our lab via UPS ground service.
- 12 month no-fault unconditional guarantee - Our lab will remake your case
at 50% of the current fee no matter what the reason for 12 months after invoicing.
- Laboratory On-Line - Track your cases, keep up to date on your account
transactions and order cases in advance for fastest turnaround times.
